Ordering Info Order by Mail (Money Order, Certified Bank Check)
When paying by money order or certified bank check, select the pay-by-check option, then continue with the ordering process. When you get to the field asking for a check number, put in the money order/bank check number instead (if you have it -- if not, enter 12345, and we'll know you're sending a money order). Print out two copies of the final invoice: keep one copy for your records and mail the other one to us, along with your money order/bank check, to the address below.

Order by Fax
If you wish to place an order by fax, check off the pay-by-check option on the order form, then continue with the ordering process. Print out a copy of your order after it's completed.
If you're sending us a money order or certified bank check, just fax the order form to us at 1-508-477-3500. You may fax us at any time; the fax machine is on 24/7. We'll process your faxed order when we receive your check in the mail.
If you're paying by credit card (MasterCard or Visa), you'll need to add your credit card information to the bottom of the order form printout you intend to fax to us: write in your name as it appears on the credit card, the 15- or 16-digit credit card number, and the expiration date, as well as your signature. You'll also need to supply us with the last three digits of the long number that's printed in the signature line on the back of your card (Visa, MC). Fax us the copy that contains the credit card information and keep the other one for your records. Our fax number, which is available 24/7, is 1-508-477-3500.
